Assessment Guidelines

  • PACs regardless of frequency: Grade 1. If symptomatic, consider starting a beta-blocker or calcium antagonist and performing routine non-cardiac diagnostics.
  • PAT: Grade 1. If symptomatic, consider starting a beta-blocker or calcium antagonist and performing routine non-cardiac diagnostics.
  • Incessant atrial tachycardia: Grade 3.
  • PVCs ≤ 5%: Grade 1; if symptomatic, consider starting a beta-blocker or calcium antagonist and performing routine non-cardiac diagnostics.
  • PVCs > 5%: Grade 3.
  • Triplet: Grade 2, with further assessment based on clinical context:
    • Age > 80 years (Grade 1)
    • Rest (Grade 1) vs. exertion (Grade 3)
    • RR interval > 375 ms (Grade 1), coupling interval (short = Grade 3)
    • Monomorphic (Grade 1)
    • Polymorphic (Grade 3)
    • Medication, Symptoms, etc.
  • AF / Atrial flutter: Grade 3 (consider RTA-AF guidelines) unless previously known.
  • Wenckebach block (AV block type Mobitz I):
    • Asymptomatic (both day and night): Grade 1.
    • Symptomatic: Grade 3.
